Healthcare professionals are required to submit vaccinee particulars to the National Immunisation Registry (NIR) for surveillance. A registry of all patients vaccinated will enable MOH and the Health Sciences Authority (HSA) to keep track of the number of doses administered and calculate the incidence rate of any adverse event that may occur.

Click on "Access NIR to notify H1N1 vaccination" under "Applications". Please follow the detailed step-by-step instructions provided on the NIR website. Download the excel template to compile patient details for uploading onto NIR registry. Record of up to 100 patients may be submitted each time.
